What to expect at your first appointment

At your first consultation, Dr. Phan or Dr. Chan will review your dental and medical history, as well the status of your current condition, in order to formulate a customized treatment plan for you. This includes determining the surgical and anesthesia options that are right for you. During your visit, your surgeon will also examine your mouth; take x-rays, or perhaps take a computed tomography (CT) scan, if needed. 

Your consultation doubles as valuable time with your surgeon, so you should use this time to ask questions and learn more about your procedure. Our caring doctors and staff take the time to explain every detail of your surgery and will address all of your questions and concerns, no matter how small. We understand that oral surgery may make some patients feel uneasy; therefore our goal is to provide you with as much information as possible to help you feel more at ease with your upcoming procedure.

When you arrive at our office

When you arrive at Contemporary Implant and Oral Surgery for your consultation appointment, you will be greeted by our friendly front desk staff. The front office receptionist will instruct you to fill out patient registration forms if you have not already done so online. We highly recommend that patients fill in their paperwork online before arriving to our office. If your forms are completed upon arrival, our team will take you to meet with either Dr. Phan or Dr. Chan.

Patient Registration

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

 

 

What should I bring with me to my consultation?

New patients should bring the following to their consultation:

  • Your Id card (drivers license or state issued ID card)
  • Your insurance card
  • Your medication list (if applicable). 
  • Your referral slip from your dentist (if one was provided).
